The D is actually DaddE for the first couple bars, then Am7/D. And
those horn chords aren't D7#9 at all; rather, they're built up in fourths
from F:

F4/7 (F-Bb-Eb) x888xx
F7#9sus4 (F-Bb-Eb-Ab) x8889x

The jazz break on the LP version is E7#9-A13-D7#9-G13 repeated several
times, then E7#9-Eb7#9-D7#9.

And then the chord progression at the end is

E7#9 A13 [Eb] D7#9 Gmaj7

where the [Eb] is a single note played on trombone.
